To request a name change, please log into your customer profile and select "Manage Profile/Preferences", select "Request a Name Change" complete the request form and attach a copy of your current government-issued ID. Submit your request and we will update your full name as it appears on your ID. Name changed due to a marriage or divorce, please provide a copy of your marriage license if your id has not  been updated.
